Federal rules require a DOT medical certificate if you: operate a vehicle with a gross weight rating over 10,000 lbs across state lines, transport 9 or more passengers for compensation (or 16+ without), or haul hazardous materials requiring placards.
Many intrastate Ohio drivers also need certification under state rules, and many employers require it regardless. If your CDL or your employer says you need a med card, you need a DOT physical from a certified examiner — regular doctors' notes don't count.
