No appointment necessary. Sports physicals are a walk-in service at our Forest Park clinic, Monday through Saturday. Bring your league or school form and we'll complete it during the visit.

The exam covers everything OHSAA and club leagues require: vitals, vision, cardiac screening questions, musculoskeletal assessment, and clearance determination.
