School entry physicals at Daystar are $50, all-inclusive: growth measurements, vision screening, blood pressure, heart-lung exam, immunization record review, and your district's form completed and signed before you leave.
Ohio requires physicals for kindergarten entry in many districts and for all interscholastic athletics. August is our busiest month — beat the rush by coming in early summer.
Bring your child's immunization record and the school's form. Most visits take 20–30 minutes.
